Gynecomastia surgery: Techniques & Alternatives

Assessment or treatment of an underlying cause may come before cosmetic surgery. Weight management can change a fatty chest component but does not necessarily remove glandular enlargement. Observation may be appropriate in some situations. If surgery is suitable, fat removal, gland excision and skin removal can be considered separately rather than bundled without explanation.

Start with the treatment goal

Male breast enlargement can involve glandular tissue beneath the nipple, surrounding fat or a combination. Significant enlargement may also stretch skin and alter areola position. A surgical plan should distinguish those components instead of treating every chest as a liposuction case. The aim is a more proportionate contour while avoiding excessive tissue removal or a new hollow beneath the nipple.

Liposuction

Treats a fatty component through small access incisions.

Glandular excision

Removes glandular tissue when fat removal alone would be insufficient.

Skin removal

Addresses substantial excess skin and may reposition the nipple-areola complex, with additional scars.

Combined treatment

Addresses several tissue components when the examination supports it.

Comparing the trade-offs

Liposuction removes fat but may not adequately treat firm glandular tissue or excess skin. Removing more tissue is not automatically better, since contour and nipple position also matter. Surgery does not prevent recurrence from an unresolved cause. A muscular appearance cannot be promised solely through tissue removal, and existing chest-wall asymmetry can remain visible.

Which approach fits your circumstances?

An assessment should review when the enlargement began, whether it affects one or both sides, weight changes, medicines and supplements, and any relevant medical conditions. The surgeon should establish that the condition and goals are suitable for surgery. Patients need realistic expectations about asymmetry, scars and the effect of future weight or hormonal changes.
