Assessment should connect the concern to muscle activity and review medical conditions, medicines, previous injections and any past unwanted response. Mention swallowing or breathing problems, neuromuscular conditions, pregnancy or breastfeeding. The clinician must decide whether treatment is appropriate and which muscles can be treated without interfering with the function or expression you want to preserve.

Targeted muscle treatment: The plan identifies the specific muscles responsible for the concern and the movement to preserve.
Observation or skin-focused care: When the concern is not mainly muscle-driven, discuss a different target or no injection rather than adding more areas.
Discuss risk before consent
Bruising, discomfort, headache, unwanted weakness, asymmetry and drooping of nearby facial structures can occur. Rare effects can extend beyond the intended muscles. New difficulty breathing, swallowing or speaking requires urgent medical assessment. The injector should explain how to report an unwanted response and when an in-person examination is necessary.
Agree the follow-up plan
Small injection marks, temporary redness or bruising may be visible even when the appointment is brief. The response develops after treatment rather than being fully assessable at the chair. Follow the injector’s instructions about touching the area and activities, and arrange the appropriate review point. Do not add another clinic’s injections before the first clinician has assessed the response.
